Location: The post holder may be required to work at any venue/establishment across the Black Country and West Birmingham, however, will in the main work from home.
Hourly Pay Rate: Band 8A (£24.10 to £27.22 depending on experience.
Contract Length: 12 months
Hours: 37.5 hours per week
Grafton Recruitment are now seeking to appoint a Project Manager to support the Children & Young People's Transformation Programme on behalf of our client-based in West Birmingham. The Project has been designed to address unwarranted variation locally in the provision of the following Long-Term Conditions; Asthma, Diabetes, Obesity and Epilepsy to ensure best practice pathways are evidence based, improve productivity, drive up the quality of services and improve outcomes thus addressing health inequalities and achieving the ambitions of the Long Term Plan The role is for 12 months and is available immediately.
The post holder will project lead the initiative outlined above, including:
Identify the local priorities
Ensure that Promotion and prevention is at the heart of any pathways amended/developed
Ensure that we can demonstrate reduced acute hospital contacts
Apply 'Right Care at the right time at the right place' philosophy
Formulate plans in partnership with other stakeholders
Define monitoring metrics in line with the Long-Term Plan and the local priorities
Ensure the voice of the child, young person and their guardians is heard
Develop collaboration across Heath, Social Care, Primary Care and Local Authority
Develop joint commissioning where appropriate across pathways
Provide assurance and updates as requested to NHSE/I
Good communication skills with ability to work across organisations
Project Management experience within the Public Sector.
Understanding of the Children & Young People's (CYP) agenda, and/or Long-Term Conditions
Commissioning knowledge especially in-service transformation, able to demonstrate previous experience of developing services to support driving efficiencies in services and improving outcomes
Ability to prepare and produce concise communications for dissemination to stakeholders as necessary
Computer literate with developed spreadsheet and word processing skills
