Assistant Public Health Practitioner Advice Line

Job description

To support the Healthy Family Team in managing incoming and outgoing information via the telephone system and utilising SystmOne for recording information.  

Main duties of the job

The post holder will perform all functions and tasks as required for the advice line team.
The Practitioner will take responsibility for the daily information flow via the telephone, including RECAP with liaison with B2 practitioner.
The post holder is responsible for the smooth transition of information from families and others to the Healthy Family Team.
The team will be working proactively using a partnership approach to promote health and contribute to tackling health inequalities within the level of competencies required for the post.
A key aspect of the role is to work flexibly to meet the needs of the service as required.
