Community Connections Lewisham (CCL) is a preventative Social Prescribing project, delivered by Age UK Lewisham and Southwark (AUKLS), aiming to improve the health and wellbeing of vulnerable adults across the borough of Lewisham.
We support people to find:
The role will require managing and prioritising your own caseload, in accordance with the needs, priorities and support required by your clients. It is vital that you have a strong awareness and understanding of the various and complex needs faced by vulnerable adults in the community, such as financial hardship, loneliness and social isolation, physical and mental illness.
This role allows you to be creative in providing opportunities for a person to develop a sense of belonging within their community. You will support them to set goals, grow their skills and improve their confidence. You will attend community groups and activities within your allocated Neighbourhood, to learn about the type of activities and support available to your clients and the difference this could make on their wellbeing. You will use these as opportunities to promote CCL support to their clients too.
This is a busy and rewarding role giving you the opportunity to make a real difference to the lives of some of the most vulnerable adults in our community. You will have the opportunity to work alongside Adult Social Care, and gain a greater understanding of local VCS organisations and community groups. You will be working closely with the wider team, taking an active part in the direction and development of the service, and contributing to other areas such as the Gaps Report, fundraising and more.
