A new and exciting opportunity to work within the Bridgewater Community Healthcare NHS Foundation Trust in a new role called the Community Health and Wellbeing Worker.
We want you to work in the heart of our community and help improve the health and wellbeing of the young, the old and their families. Community Health and Wellbeing Workers are passionate role models and provide advice and signposting to everyone within their communities, enabling them to make positive choices around health and wellbeing.
Youll meet people face to face, providing support and advice to those that need it. Youll help people find local health services, offering to help them make appointments or sign post them to local services to support their needs.
Were looking for people who are computer and phone literate to help the local population get connected to good quality, online health information.

Dont worry about having previous NHS experience because you dont need it well be giving you full, on the job training, as you help us to deliver support to the local communities in Warrington.

The post is currently funded until March 2024 with the potential for extension.

Main duties of the job

Community Health and Wellbeing Workers will champion health and wellbeing in their local area by raising awareness, signposting people to local services and resources and provide support to remove barriers to access. They will play a significant role in increasing health promotion, through a range of activities including outreach assessments, community education and advocacy.
They will contact households in a defined area on a regular schedule of visits, face to face or virtually as appropriate, listening and discussing health needs on each visit. They will identify vulnerable households or individuals in conjunction with health care teams and identify health and/or social care needs in conjunction with health care teams.
The Community Health and Wellbeing Worker will act as an advocate to help households navigate the health and social care systems, access appropriate services and remove blocks to accessing services and resources.
They will give healthy lifestyle advice including advice on immunisation and screening following appropriate training.
