Health Coach / Youth Worker

Job description

Diabetes outcomes are seen to particularly decline when children and young adults with diabetes transition to adult care. 4YP are supporting Ipswich Hospital to be a site for NHSE’s ‘Diabetes Transition Pilot’. Currently, the Transition service provision at Ipswich Hospital Diabetes Centre covers 16-25 year olds. 4YP is seeking an experienced youth worker to be a part of our youth work team as a ‘Diabetes Health and Wellbeing Coach’ - a youth work and social prescribing post dedicated to working with this cohort of young people.
Occasional evening work per week is an expectation of this role.
