Job description

An exciting opportunity has arisen for a Population Health Portfolio Manager to join the Population Health Care team at Lewisham and Greenwich NHS Trust (LGT), where they will play an integral part of the Population Health & Care programme team.

Main duties of the job

Develop and maintain collaborative relationships with a broad range of senior system stakeholders to define and implement transformation projects

Produce and maintain programme and project plans for work requests to ensure timely delivery of agreed milestones and objectives

Identify and manage portfolio risks, issues and dependencies to ensure successful delivery of projects and minimise disruption to the programme

Deputise for Associate Director where appropriate, including but not restricted to stakeholder engagement, escalation meetings, working group facilitation, product demonstrations and liaison with Oracle Cerner

Lead and engage local project and clinical working groups alongside Population Health Data Analytics workstream, ensuring all members understand their roles and responsibilities
