Since 2021, Imperial Healthcare NHS Trust and Chelsea Football Club Foundation have had an exciting partnership focussed on improving health and addressing inequalities and we are looking for a kind, ambitious and enthusiastic team member to take on the role of population health project manager to deliver initiatives which work at the intersection between sport, health and well-being.
The role will involve delivering and enhancing the existing provisions that have been developed between the two organisations, supporting the co-production of new health and wellbeing initiatives and evaluating their impact.
This post will be based across both Imperial College Healthcare NHS Trust and Chelsea Football Club Foundation. By the end of this fixed term contract it is hoped that the individual will have embedded initiatives into our communities, developing a clear evidence base which facilitates the commissioning of these services by the NHS and demonstrates the impact of partnerships in health and sport.
The role provides an opportunity to design and deliver work which addresses health inequalities in North West London and improves the health and wellbeing of patients, local residents and staff. We are looking for an individual who can bring their experiences in life and work to help us improve outcomes across a population using innovative approaches, as well as someone with an understanding of the role other sectors, such as sport, can play in supporting health and care.
Working closely with key populations to deliver initiatives that tackle health and wellbeing issues that are most relevant to our patients, staff and local communities
Developing strategies and approaches to evaluate these initiatives using both qualitative and quantitative methodologies
Creating a communications and engagement strategy to maximise uptake of these initiatives and impact of the broader strategic partnership
Manage the partnership between ICHT and CFCF and work closely with Imperial Health Charity, to oversee the deliverables and identify new opportunities to pursue together
Collaborating with CFCF to review existing health and wellbeing activities, identifying service gaps and themes for mutual growth and development.
Using insights to develop an approach to sustaining this work including identifying funding opportunities and developing an approach to financial sustainability
