An exciting opportunity has arisen to appoint inspirational and enthusiastic Staff Nurses to work as part of the integrated 0-19 Children & Family Health Teams within the Blackburn & Darwen area to deliver the Healthy Child Programme, universal and targeted provision to children aged 0-19 and their families.
The 0-19 Children and Family Health Teams are proactive and forward thinking and have introduced a new model of service delivery for school age children up to age 19 and 25 for those young people with special educational needs and disabilities. Successful applicants will work in one of 2 specialisms, one focusing upon the delivery of the Healthy Child Programme and one focusing upon Safeguarding. Working as part of team under the delegation of the School Nurse your role will require excellent assessment skills, partnership working and a strong commitment towards delivering public health and health care support to school aged children.
The area of Lancashire care foundation trust footprint covers a wide area and you will be expected to be flexible to meet the needs of the service; however, localities often remain the same.
The post holder, as part of the 0-19 service, will work with school aged children, young people and their families in a variety of settings (homes, schools, communities) to deliver the Healthy Child Programme 5-19 (25)years.
Under the direction and leadership of the Specialist Public Health Nurse (SCPHN – SN/Registered School Nurse) the post holder will, provide a family centred, evidence based public health service to a defined caseload, population or community, which promotes and improves health, addresses inequalities and safeguards children and young people. This will be achieved through the delivery of a public health nursing model at a Community, Universal, Universal Plus and Universal Partnership Plus level. The post holder will work in line with the Trust’s corporate objectives and will be expected to make a positive contribution to the provision of health services. The post holder will be expected to work in line with the Trust values delivering excellent care with compassion.
