Public Health Strategist - Population Health Management

Job description

Delivery of Population Health Management in North Central London (NCL) is supported by Cerner’s HealtheIntent platform. HealtheIntent is being used to link health and care data from across the 22 organisations within NCL’s Integrated Care System (ICS), to create a longitudinal, normalised health and care data set. These data will be used to support the delivery of direct care across the system, particularly where care needs to be more integrated, and to improve population health outcomes.A key part of the HealtheIntent programme is the scoping, development and implementation of a suite of new population health management (PHM) registries. This work is led by a Population Health Intelligence Lead based in the ICS’s centralised intelligence team. The planned suite of HealtheIntent registries includes a wide range of conditions and attributes of importance to both health and social care, across the life course.We are looking for a Public Health Strategist to help deliver the HealtheIntent ambitions in Camden and Islington. The postholder will support the Population Health Intelligence Lead responsible for the HealtheRegistries part of the HealtheIntent programme. Since this programme is being implemented across North Central London, the postholder will need to develop local solutions that are applicable, where possible, to the other boroughs in North Central London. This is not primarily a technical role, rather a strategic role that is able to link effectively between front-line NHS and care staff, Public Health Analysts and Strategists and Cerner colleagues. Key responsibilities include:Leading on using HealtheIntent to identify and address health inequalities in Camden and Islington.Supporting the development of HealtheRegistriesSupporting the maintenance of (and where appropriate building on) work developed during the COVID-19 pandemicSupporting / leading on implementing new registries needed
