St. George’s University Hospitals NHS Foundation Trust is seeking a Smoking Cessation/ Tobacco Dependence Link Worker to support the delivery of tobacco dependence treatment within the Trust.
The NHS Long Term Plan tobacco objectives set out clear commitments for NHS action to improve prevention by tackling avoidable illness such as Tobacco Dependency by supporting patients, service users and staff to overcome their tobacco dependence. This will not only provide improvements in their health but reduce health inequalities and decrease demand on services by reducing the number of smoking related admissions and readmissions to hospital.
There will an opportunity for **2 posts (full time, part time) **to be based in the Maternity service but there will be an expectation for Trust-wide training/working to cover the other Trust Link workers.
Main duties of the job
The successful candidates will join our Maternity team and:
Work with the Smoking Cessation/Tobacco Dependence Treatment team to deliver behavioural stop smoking support throughout the service area according to national and local standards and assist in the delivery of outreach.
Develop and sustain referrals into the service, to deliver advice to inpatient smokers and pregnant smokers on an opt out basis and to support patients to make personalised tobacco dependency care plans.
Motivate and assist inpatients, outpatients and staff within the Trust to make informed choices in stopping smoking and be involved in the provision of relevant professional advice and expertise in matters relating to smoking cessation.
Work with specialist groups including pregnant women, those with mental health needs and those with acute health conditions.
Report on interventions and outcomes on a regular basis.
Assist in developing the service and promoting a culture change towards the Trusts approach to smoking.
