Buckinghamshire Mind is seeking a resourceful, relationship-building, Social Prescriber to deliver our Social Prescribing service which empowers people to take control of their health and wellbeing. Through GP referral you will connect people to community groups and statutory services for practical and emotional support, working collaboratively with all local partners.
Do you have the following key skills for this role?
· empathic and active listening skills
· experience of supporting people with their mental health
· the ability to ‘join the dots’ as part of a team in a collaborative project
As this role is based in the community with travel between several locations, you’ll need access to a vehicle for use during work hours. Alternative transport arrangements will be considered.
