An exciting opportunity has arisen to recruit a second SPLW to joining our already established service, the applicant should be an enthusiastic and empathetic individual, to work in Northolt PCN - we comprise of 6 practices, working together to provide the best possible care to our patient population - We are an innovative, supportive and friendly team and look forward to having a a second Social Prescribing Link Worker join us.
Social prescribing empowers people to take control of their health and wellbeing. Non-medical link workers give time to individuals and focus on what matters to me they take an holistic approach, connecting people to community groups and statutory services for practical and emotional support. Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners. The Social Prescriber post holder will be working in partnership with the existing SPLW and the practice teams in their Primary Care Network (PCN) to support patients. The Social Prescriber will have a good understanding of social prescribing, well-being and community services together with a good understanding of and ability to provide motivational and behavioral techniques.
