Social Prescribing Link Worker – Oldham PCN at Action Together

Job description

Action Together are recruiting a Social Prescribing Link Worker to work in one of their Primary Care Networks in Oldham. The role involves working with people referred into the programme to support them to reach their personal wellbeing goals by accessing support and opportunities in the voluntary and community sector.

Action Together are looking for:

  • Someone who understands that the solutions for many of the barriers people face to improving their wellbeing can be found within their community.
  • Someone with the skills, ability, and passion to help people connect with their community to improve their wellbeing.
  • Someone who is a skilled collaborator, able to link things up to get the best outcome, and able to proactively seek to develop their knowledge and skills, but who is confident in working alone and can manage a busy caseload.
    Contract: 12 months fixed term (with potential to extend)
    Please note, the successful candidates will be required to undertake an enhanced Disclosure and barring Service (DBS) check.
