Reports to: Community Prescribing and Development Lead
Duration: Fixed term contract for 1 year
Place of Work: Atlantic Medical Group Clinics in Penzance with the requirement to work at different locations and venues in Cornwall and The Isles of Scilly. Due to the nature of the work, travel is an essential part of the role therefore a driving license and access to a car is a requirement. Your car insurance must cover business use. Mileage expenses are claimable.
Salary: £25,969 per annum (pending pay review)
Hours of Work: 37.5 hours per week

About the role:
Social Prescribing Link Workers are key health care practitioners working in General Practice alongside the multi-disciplinary healthcare team. Receiving referrals from a multi-disciplinary team of health professionals and other agencies. Social Prescribers give people time, focusing on “what matters to me” Taking a holistic approach providing 1:1 personalised support empowering people in improving their Health and Wellbeing. Connecting people to statutory services for practical and emotional support.
Working in the Centres hosting consultations and conducting home visits will be key in developing trusting relationships with people, creating shared personalised care and support plans together, connecting people to community groups and other Voluntary Community and Social Enterprise organisations (VCSE) and services.
Social Prescribing Link Workers support people in gaining greater control over decisions and actions affecting their health in improving their Health and Wellbeing. Identifying wider determinants upon their health, such as debt, poor housing and physical inactivity, low-level mental health concerns as well as other lifestyle issues, by increasing people’s active involvement within- their local communities. This approach particularly helps with long term conditions, people who are lonely or isolated, or who have complex social needs which affect their wellbeing.
There is growing evidence that Social Prescribing can lead to a range of positive Health and Wellbeing outcomes for people, such as improved quality of life and emotional wellbeing. The NHS long term plan was ambitious by incorporating Social Prescribing into its comprehensive model of personalised care.
About you:
The successful candidate will have experience of working positively with people facing complex social and emotional challenges. Be a good listener, confident, possess great interpersonal skills, well organised, computer literate with good IT skills. Have time for people and be committed to supporting local communities to care for each other.
