Social Prescribing is a means of enabling primary care services to refer patients with social, emotional or practical needs to a range of local, non-clinical services. This service is provided as part of a model of partnership between the voluntary, community, faith and social enterprise sector (VCFSE) and GPs.
The role is to develop a high quality customer care service to Multi-Disciplinary Teams and Patients in the Grays PCN area a within Thurrock and to be responsible for the liaison and development of effective working relationships with health professionals and GPs. You will work alongside voluntary and community groups to provide patient support for GP practices.
The role is funded by NHSE through the PCN’s. Social prescribing works for a wide range of people including; with one or more long-term conditions, who need support with their mental health, who are lonely or isolated, who have complex special needs that affect their wellbeing.
We will evaluate applications as we receive them, reserving the right to close the post should we receive a high volume of suitable applicants.
