The role of the Social Prescribing Officer is to work as a key part of the established Action Together Community Wellbeing Programme to support people in Tameside to play a more active role in managing their health and wellbeing by accessing community-based provision that improves their community connections and supports them to improve their personal resilience.
Action Together are looking for someone who is committed to helping people improve their own wellbeing through nonmedical community-based support from the VCFSE.
The ideal candidate will be:
A gifted communicator, able to work with all sorts of people with very different aspirations, limitations, and personal circumstances, recognising their strengths and helping them to improve their wellbeing through non-medical support in their community
Someone who is skilled at finding out what is available to help the people they support
A team player, committed to collaboration who can develop and maintain excellent relationships internally and externally to get the best possible outcomes for the people we all support
Someone who genuinely understands the role that the VCFSE has in supporting people with all aspects of their lives
Someone who can work under pressure while maintaining their own wellbeing and continuing to give clients and stakeholders the best support possible
An organised individual who recognises the need to record, monitor and evaluate their work using the systems and processes available to them
A genuine commitment to inclusion and equality, driven by social justice
Can manage their own workload and meet agreed timescales and outcomes in-line with service standards and approach
Is outcomes driven and able to flex and prioritise to meet agreed commitments
Length of contract: 12 months fixed term maternity cover
