The Health Inclusion Team provides an innovative nursing and health visiting service to a transient population. That is; people who live in homeless accommodation, women's refuges, asylum seekers, refugees, Gypsies and Travellers.
As a Specialist Health Visitor you will be expected to undertake family health assessments and child health surveillance in accordance with the Healthy Child Programme for adults and children, 0 to 19 years.
Working in partnership with children, families and individuals to provide high quality effective healthcare, to prevent ill health, promote positive health and reduce health inequalities. You will contribute to the development of the service for Homeless people, Gypsies and Travellers and Asylum Seekers and Refugees.
The post holder will need to be a Registered Specialist Community Public Health Nurse (Health Visiting) and a Community Practitioner Nurse Prescriber with current NMC registration. In order to undertake the duties of the post the successful candidate will have relevant experience working with diverse communities and socially disadvantaged groups.
The team works closely with a range of agencies to ensure a strong and coordinated service working within the NMC Code of Professional Standards and Clinical Governance Frameworks.
