Wellbeing Coordinator - Better Leeds Communities

Job description

Linking Leeds exists as a consortium of seven local partners with over 10 years’ collective experience delivering Social Prescribing for Leeds brilliant and diverse communities. These partners include Community Links, Barca, Leeds Mind, Feel Good Factor, Age UK Leeds, Better Leeds Communities and Leeds Irish Health and Homes.
Linking Leeds aims to improve physical, mental and social health and wellbeing, through a holistic approach to improve quality of life and confidence in self-management and supporting independence.
We aim to provide high quality services adhering to principles of best practice, promoting equal opportunities and working positively with diversity. All posts have two main functions: to carry out the duties as outlined and be proactive in continuously improving service delivery.

The Role
** **As a member of the Linking Leeds team you will make a real difference to the lives of others. You will be a keen advocate of the benefits of supporting people with health and social care needs to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.
The post holder will support individuals to build independence and personal resilience, enabling self –management of their health and wellbeing. This will be achieved through the creation of Wellbeing plans which will be tailored to individual need. Key to the role will be the ability to build trusting relationships quickly whilst motivating individuals to make sustained changes, working sensitively and overcoming barriers to access.
Wellbeing Co-ordinators will build and develop relationships with a wide range of services and activities in order to address social and health care needs, signposting and offering support to individuals to participate and benefit from community-based activity.
Wellbeing Co-ordinators will be integrated as part of Primary Care Networks to improve the way the needs of local populations are met.
This is a unique and exciting opportunity to play a valuable role in the development of the city-wide, service delivering innovative approaches in a health care setting.
The roles available are based over various sites including community venues, GP practices, and occasional work from home. The Linking Leeds office is The Reginald Centre, 263 Chapeltown Road, LS7 3EX.

1 x 37 Hours per week
2 x 18.5 Hours per week
1 x 34 Hours per week
1 x 28 Hours per week
Please note that we are recruiting on behalf of the Linking Leeds consortium. Posts available I this recruitment round will be with Barca Leeds, Leeds Mind and Better Leeds Communities. The terms and conditions, including salary and staff benefits will be variable, depending on employer. The Salary range is £24,000 to £27,799.
